Real Comfort Living LLC is part of the Home and Community-Based Services(HCBS) program that provides individualized care for adults with mental health conditions. Through this program, we deliver services that help clients integrate into the community, stay in their home
and receive the support they need for behavioral health management.

Primary diagnoses of serious mental illness

Age 18 years or older

Income below 150 percent of the federal poverty level

Able to live in a community with program services

Actively receiving medicaid or medical-eligible if discharged from a state hospital
